We are open to new registrations from within our Practice Area. If you require a new patient health check at point of registration, please inform the reception staff.
Ask to register for Appointments Online.
You will be given a unique username and password - along with details of how to gain access to the website. To register for this you need to complete the Patient Online Access form (see below). We do require proof of ID to register you for this service. Click here to download the Patient Online Access form
If you experience any problems using this service contact the surgery for assistance.
Patients who are 16 and over and under 18 years of age
You have now reached the age where you are entitled to confidentiality.
You may wish to have your own contact details on your records. This will mean that when the surgery contacts you we will be speaking directly to you rather than involving someone else in your care.
If you wish to keep the same family details this is fine and the surgery will always respect your confidentiality.
Documents Requested For Registration
- Passport (if applicable)
- Asylum seeker card (if applicable)
- Proof of name & address e.g. Utility bill (less than 3 months old), Council tax bill, driving licence, bank statement
- Completed health questionnaire
- Children under 16 cannot register without guardian/family
- Red book (for all children under 6)
If your circumstances mean that you are unable to provide the above documentation. Please let the practice know when you provide your completed registration forms.
Please note: All new registrations require the completion of the GMS1 Form * Registrations cannot be processed without this.
